It started out as just a paint job, but the more I got in to it...  Well you know how that goes!
I discovered much more rust in the cab than expected, so I'm pretty sure I'm going to swap it out.  I've been junkyard shopping all summer and haven't found one yet that will justify a swap.  I decided to powder coat the frame as that seemed like much less work than cleaning, priming, painting, etc.  However, one of the rear suspension blocks got cracked in the coating process.  They were able to repair it by welding.
